What's New for 2026
The 2025-2026 respiratory virus season remains dominated by the triple threat posed by COVID-19, RSV and influenza. Measles outbreaks are a continuing concern. Throughout this edition we have updated treatment recommendations based on recent references and guidelines. Also, we have removed many older references that have been superseded or are simply no longer necessary.
Major updates include:
Table 1—major updates to native valve endocarditis, adult arthritis, complicated urinary tract infections and plague, as well as minor updates to regimens and comments throughout
Table 3—duration of therapy, is substantially revised
Tables 5 and 6—highly resistant bacteria, are substantially revised
Table 7—new desensitization protocols and references added
Table 10A—gets recently approved anti-infectives
Table 12A—pulmonary tuberculosis has a major update
Tables 11, 13 and 14—covering fungal, parasitic and viral infections respectively are updated as needed
Table 15—prophylaxis for dental procedures and candida and cocci prevention are updated for improved clarity
New tables 18A, 18B—cover hepatic impairment dosing and DRESS
Drug information tables covering pediatric dosing, pharmacology, enzyme- and transporter-mediated interactions, renal impairment dosing, obesity dosing are updated throughout.
Some recommendations may suggest the use of antimicrobial agents for indications or in doses that differ from the approved full prescribing information. Any such recommendations are based on published reports in peer-reviewed literature with citation to evidence-based references. They are not made based on input from any manufacturer and are made with due consideration for the concerns of the U.S. FDA regarding “off-label” uses.
